Loose Pivot bolt anyone else had the issue ?

Discovered today, whilst washing my 1250RT, that the pivot bolt (as per photo below), was loose and the serrated washer turned easily. I tightened it back up to 145 Nm, which is the recommended torque so wouldn't expect it could easily loosen. Anyone else had this issue ? Less than 1000 miles ago, the driveshaft was replaced under warranty at a BMW dealers. Would this involve removing this bolt ?

Sounds like the tech forgot to torque the bolt
With that torque, it's not going to go loose by itself
